Notice of a Data Incident

Michigan Orthopaedic Surgeons (“MOS”) recently discovered a data incident that may have involved certain individuals’ personal information. On or about June 29, 2023, MOS identified suspicious activity in an employee email account. Upon identifying the issue, MOS promptly secured the account and began an internal investigation. MOS also engaged a forensic security firm to assist with its investigation. The forensic investigation determined that an unknown, unauthorized third party accessed the email account between May 5, 2023 and June 21, 2023.

MOS conducted a comprehensive review of the contents of the account to determine if they contained any personal information. This comprehensive review took time and on October 20, 2023, MOS determined that the account contained personal information including, depending on the individual, their name, date of birth, Social Security number, financial account number, username and password, health insurance information, and medical information, such as diagnosis, lab results, and prescription information. On December 19, 2023, MOS began mailing notification letters to those individuals whose information was in the account for whom MOS has a valid mailing address.

Individuals should refer to the notice they receive in the mail regarding steps they can take to protect themselves. MOS has arranged for complimentary credit monitoring for the individuals that had their Social Security numbers potentially involved. As a precautionary measure, individuals should remain vigilant to protect against potential fraud and/or identity theft by, among other things, reviewing their account statements and monitoring credit reports closely. If individuals detect any suspicious activity on an account, they should promptly notify the financial institution or company with which the account is maintained, and report any fraudulent activity or any suspected incidents of identity theft to proper law enforcement authorities, including the police and their state’s attorney general. Individuals may also wish to review the tips provided by the Federal Trade Commission (“FTC”) on fraud alerts, security/credit freezes and steps that they can take to avoid identity theft.
